About the Role:
Make a meaningful impact by supporting children and families referred by the Department of Children and Families. In this dynamic, community‑based role, you’ll build trusting relationships, conduct home visits, connect families with vital resources, and help strengthen family functioning using a strengths‑based, family‑driven approach.
What You'll Do:
- Build strong rapport with children, parents, caregivers, and community partners
- Guide families through transitions as they enter the program
- Conduct needs assessments and develop individualized, strength‑based Plans of Care
- Facilitate Family Team Meetings with both formal and natural supports
- Collaborate with DCF, schools, healthcare providers, and community agencies
- Provide home visits and deliver tailored interventions to strengthen family functioning
- Ensure effective communication among families and service providers
- Collect and report engagement surveys, assessments, and program data
- Attend community meetings and represent the program professionally
- Use your own vehicle to visit families and occasionally transport clients

Qualifications
- Bachelor’s degree in Human Services from accredited post secondary institution.
- Minimum one year experience working with children and families in diverse populations.
- Bi-lingual (English/Spanish) preferred.
- Ability to work a flexible schedule.
- Sensitivity to diverse cultures and language competencies.
- Knowledge of behavioral health issues for children and families and of home-based services preferred.
- Good oral and written communication and documentation skills.
- Must be proficient in Microsoft 365. Must be able to navigate through electronic health records system (EHR).
- Ability to prioritize and organize schedules/demands.
- Must have and maintain a valid driver’s license. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization’s auto insurance plan.
- Experience working with families with complex needs preferred.
