Position: Case Manager 1

Salary: $22/hour
Schedule: Full time, 40 hours, 1st shift, Tuesday - Saturday (9 - 5p)

Program Summary:
West Village provides individualized case management and life skill building services for adults with histories of chronic homelessness and mental health and/or substance use disorders. The program's goal is to help people maintain their housing and improve their day-to-day functioning. This is an onsite (not hybrid) program.
Position Summary:
The Case Manager 1 is responsible for providing staff support and direct client care. Responsibilities include delivering comprehensive case management services to clients including intake assessments, development and implementation of individual care plans, and coordinating with treatment providers and community resources. Utilizing company provided vehicles, case managers assist clients with transportation as part of day-to-day business operations. This position also complete assessments and other required paperwork on a deadline basis. Writing thorough case notes is a fundamental aspect of this job and requires good grammar and strong writing skills. Case managers also facilitate at least one group a month. This position is non-exempt and works under the supervision of program management.
Working within Adult Services, the Case Manager 1 works to support clients’ recovery from mental illness, substance use, and homelessness.
Requirements:
A Bachelor's Degree in a human services field OR
2+ years related experience in direct service and case management field
Knowledge of resources in the Greater New Haven area
Valid Driver’s License (no exemptions)
Regular and predictable attendance is required
Orientation:
If selected for employment with The Connection, newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.
